Abstract:

Abstract:

The history of discourse analysis is beset by a vagueness around the homonym ‘discourse’. The term names a large territory, located somewhere between two ‘markers’, which might, generally speaking, be something like ‘providing accounts of connected stretches of language in use’ and ‘uncovering salient social, political, psychological features in text-like entities’. In sociolinguistics, by and large, the major emphasis has been on understanding the link between (environments of) language use and (features of) the language used . In such work, ‘the social’ and its meanings and effects are foregrounded: who speaks, to whom, when, with what purposes, in what ways. These factors and the purposes leave traces – whether the details of pronunciation in Labov’s early work or the regularity of use of a certain range of linguistic resources, leading to the development of the notion of ‘codes’ in Bernstein’s theory. In more linguistically rather than sociolinguistically or sociologically oriented approaches, the emphasis has been on seeing whether regularities of a ‘formal’ kind could be discerned in ‘stretches’ of speech and writing ‘above’ the sentence, somewhat akin to those that linguistics had been able to establish in relation to the sentence in the 1970s – whether in mid-century American structuralism or in Chomskyan conceptions of the organization of language at or below the level of the sentence.

Abstract:

Aims: This study was conducted to provide a comprehensive analysis of multimodal pain prevention strategies, focusing on their effectiveness in managing and preventing various types of chronic pain. Method and Materials: In this systematic review comprehensive search of electronic databases, PubMed, was conducted to identify relevant studies published up to January 2024. The search strategy included terms related to multimodal pain prevention, such as "pain management, " "non-pharmacological interventions, " and "integrated pain care. " Studies were included if they evaluated multimodal pain prevention strategies in various clinical populations and reported outcomes related to pain intensity, functional status, adverse effects, and patient satisfaction. The methodological quality of the included studies was assessed, and data were synthesized to provide an overview of the findings. Findings: The search yielded a total of 6 studies that met the inclusion criteria. The included studies encompassed a diverse range of patient populations, focusing on those experiencing chronic pain and receiving palliative care. Multimodal pain prevention strategies were found to be associated with improved pain control and reduced drug consumption in patients experiencing chronic pain. Furthermore, non-pharmacological interventions, such as physical therapy and cognitive-behavioral therapy, were shown to enhance pain management outcomes in various clinical contexts. The integration of these modalities in a coordinated and individualized manner appeared to be particularly beneficial in addressing the complex nature of pain. Conclusion: The findings support the use of multimodal approaches in clinical practice and emphasize the importance of individualized, patient-centered care in pain management. Future research should focus on identifying the most effective combinations of interventions and elucidating the mechanisms underlying the synergistic effects of multimodal approaches.

Abstract:

Purpose: To evaluate the multimodal imaging including optical coherence tomography angiography (OCTA) findings in patients with posterior microphthalmos (PM). Methods: In an observational case series, four eyes of two patients, eight and twenty-three years old, with clinical proven PM underwent complete ophthalmic examination, including refraction, fluorescein angiography, optical coherence tomography (OCT), OCTA, B-scan ultrasonography, axial length measurement using IOL Master optical measuring, and Pentacam evaluation. Results: Both patients were high hyperopic with partial thickness retinal fold in macula, retinoschisis, and foveal hypoplasia. Axial length was less than 17 mm with scleral thickening in all eyes. OCTA showed absence of foveal avascular zone (FAZ) in both superficial and deep capillary plexuses. Pentacam showed corneal steepness, shallow anterior chamber, and low anterior chamber volume. Conclusion: OCTA findings showed absence of avascular zone in both superficial and deep capillary plexuses, while OCT shows partial thickness retinal fold and retinoschisis.
